Send it, collect acknowledgments and the yes/no election, keep the proof
The template is free. Proving each employee got it, in the right language, and recording their emergency contact and detention-notification election is the work. NoticeStamp does that with server timestamps, reminders and SHA-256-stamped PDFs.
Who gets it and when
- Every current employee, by February 1 each year (§1553(a)).
- Every new employee, at hire.
- The authorized (union) representative, annually, if there is one.
Which language
The language you normally use with the employee, if DIR publishes the template in it; otherwise English (§1553(c)).
The emergency contact part
SB 294 also requires giving employees the opportunity to name an emergency contact and choose whether that person is notified if they're arrested or detained at work (§1555). A standard form should capture:
- Employee name
- Emergency contact name
- Relationship
- Phone and/or email
- Separate election: “If I am arrested or detained at work, notify this person: Yes / No”
- Date (and, for updates, the date it changed)

Keep records for 3 years
